While the church is eternal and not going away (Matthew 16; Daniel 2), congregation come and go and go through peeks and valleys. And while the church is spiritual in nature Ephesians 4 indicates a role that leaders play in equipping and motivating for the work of ministry, the unity of the body, and the building up/growth and health of the same. That being true there are factors that can be measured. This form is designed to aid to that end. It is not perfect and it is not infallible - there are factors beyond percentages and studies at work. If you complete this form and find ways it might be more useful, let us know.
