While the church is eternal and not going away (Matthew 16; Daniel 2), congregation come and go and go through peeks and valleys. And while the church is spiritual in nature Ephesians 4 indicates a role that leaders play in equipping and motivating for the work of ministry, the unity of the body, and the building up/growth and health of the same. That being true there are factors that can be measured. This form is designed to aid to that end.
